Quick answer

Your first shift is scripted and safe: two patients that are never anomalies. Patient one wants Herbs (stomach ache), patient two wants Eye Drops (dried eyes). Use the shift to learn check-in, room treatment and the supplies shop. Cameras and shutters unlock after shift one.

The core loop

1. Patient arrives at the window — check them in (or reject them if they are an anomaly).
2. They take a medical room; run diagnostics on the computer (about 9 seconds for DNA analysis, 2 seconds for the readout).
3. The screen shows one to three sicknesses — give the matching remedy from your carried items.
4. The patient recovers (about 35 seconds) and the next one can use the room.
5. Finish the shift's patient quota to see your shift report and pay.

What unlocks, and when

You have three lives: an animal death loses one. Giving the wrong treatment kills the patient — and if they were an anomaly, the game does not count it as an animal death, but your pay drops.

Beginner tips that actually save sanity

• Grab coffee as soon as the shift starts — a fresh cup keeps brewing, and coffee restores sanity (5 per sip, 15 total).
• Always carry Herbs and Eye Drops for the scripted first two patients.
• You carry three treatment items by default; upgrade capacity or use a class with more slots.
• When a photo develops, let it finish before inspecting — picking up a cursed photo too early costs 10 sanity.
• If you tase a normal patient you lose 15 sanity and the patient's treatment is skipped — check before you tase.
• Playing solo is fine, but scan patients with the security cameras (available from shift 2) before admitting.
