Anomalies are patients that are not what they look like. Reject them at the window and your shift stays calm — admit them and events follow.
What is an anomaly?
Anomalies are animals with abnormal traits — wrong faces, wrong photos, wrong behavior. Lore in the game frames them as the result of experiments by the Animal Corporation. For gameplay, what matters is one decision: admit or reject.
How anomalies are detected
- By eye (appearance) — unnatural faces, staring, hollow eyes, twitching, hunched posture. See How to Spot Anomalies.
- By photo — the photo does not match the patient (face, ears, ear color) or shows static. Cameras are unlocked from shift 2.
- By behavior — a low distorted voice, staring through the window, odd movement between rooms.
- Some signs only appear on security cameras. No single clue is proof by itself — compare before deciding.
What happens if you admit one
Letting an appearance anomaly check in usually leads to a Death Ritual, a monster emerging, or the patient turning into a Mimic. After treatment an anomaly often says "I'm hungry..." and attacks while leaving, walks out normally, or grows tentacles (Room 8) — so rejecting at the window is the cleanest outcome.
Not every anomaly is aggressive: Hollow Face anomalies perform death rituals on themselves (stay in the room — use Eye Drops, IV Drops or coffee on the candles to avoid sanity loss), Head Banger calms down with coffee, and Barney never attacks the player.
